To explain what happened in Episode 38, we have some series of flashbacks in this episode to show the things that transpired especially between Winry and Scar.

Going back to the moment where Scar is restrained by Ed and Winry arrived wanting to talk to Scar, Winry first attended to the wounds of Scar believing that this is what her parents must have them done if they are still arrive. But Winry points out that this does not mean that that she already forgives Scar.

Miles and Scar also get to know each other and Miles shared his intention of changing people's impressions about Ishvalans which made Scar happy. May Chang and Marcoh also arrives at the scene saying that they need Scar to decipher the documents so they decided to seek refuge at Briggs. Miles asked for Scar's cooperation in exchange of delaying his trial and Scar swore on his Ishvalan blood. In order to continue fooling Kimblee that they are not planning anything, Winry suggests of having Scar kidnap her.

Kimblee's chimeras (whose names I already forgot as of this writing), are all convinced by Al to continue on living for the sake of the people they love. They realized that they all want to live to be with their own families and agreed to help them in whatever way that they can.

The plan was successful. Marcoh and his group moved through the tunnels underground. But they didn't know that they are just plunging in greater danger. Olivier was being summoned to Central and has been replaced by King Bradley's men. Miles got to know this information and it is important they he can relay them to Marcoh and his group. Al volunteered to relay the message since he can battle the blizzard because he cannot feel any pain or exhaustion. While walking the blizzard, he suddenly sees a vision of his original body infront of the gate calling out to him. The vision ends quickly and Al realizes that the rejection reaction is now happening.

True enough, Olivier arrived at Central and she suddenly sees Roy Mustang. They talked about having a meal together and florists in the city, hinting the informant Roy met previously. Olivier proceeds with her meeting the Fuhrer and realizing that she cannot hide the facts, admitted that she indeed killed Raven and expressed her intention to replace him. King Bradley, amused at Olivier's ego offered her the position in exchange of letting one of his men replace her at Briggs.

At the canteen, Mustang and Riza ate together and through their conversation, Hawkeye was able to report that Selim Bradley is a homonculus through the names they used in their conversation. Meanwhile, Father is sleeping underground and a flashback begins of what seemed to happen in the ancient Xerxes.

Back then, a slave, identified by Slave Number 23 was assigned in a laboratory to participate in experiments. In the laboratory, there's a certain being in a flask that was born from the slave's blood. The being and the slave talked and the being decided to share his knowledge to the slave in exchange for giving him his blood. He also gave the slave his own name and the name is Van Hoenheim. The being, who calls itself as Homonculus gave Van knowledge that made him become more useful to his master as an alchemist. Van is forever grateful to the homonculus for everything and started thinking of having his own family as his own happiness while the being in the flask only wanted to have a body of its own.

Some time later, Homonculus was brought to the King of Xerxes who seeks for immortality. Homonculus told him how to achieve it and the king ordered digging of irrigation channels around the country. He als ordered a genocide and nobody survived in that mass murder. After the killings, they proceeded with ritual, the king offering his own blood and the Gate opened. Everybody started dying except for Hoenheim and Homonculus who is the center of the circle. When the transmutation is over, only Hoenheim and Homonculus survived. Both of them are now immortal and inside both of them are the people who were killed as sacrifice. Homonculus also took Hoenheim's body and face.

Back in the present time, Hoenheim wakes up from this memory in a train and accidentally met with Izumi and her husband. Izumi starts coughing again and by touching her, Hoenheim was able to learn about Izumi's human transmutation attempt. He then plunged his hand in her abdomen and used his power to rearrange her organs to help ease her feeling and improve her blood flow. Izumi question who he is to which Hoenheim answered, he is Van Hoenheim, a Philosopher's Stone in the shape of a human.

Around at the same time, Ed is telling Miles that the Philosopher's Stone, the Great Elixir can take many shapes and forms. Ed further adds that he doesn't want to see a huge philosopher stone, because of the unfathomable number of people needed to produce one.
